GST/HST Attribution Point Election or Revocation of the Election for a Selected Listed Financial Institution

Fill out this form if you are a selected listed financial institution (SLFI) that is a stratified investment plan or an investment plan and you want to make or revoke an attribution point election under section 18 of the Selected Listed Financial Institutions Attribution Method (GST/HST) Regulations (SLFI Regulations) to have attribution points for the purposes of Part 2 of the SLFI Regulations for a series of the stratified investment plan or for the investment plan that are quarterly, monthly, weekly or daily.
