GST/HST Consolidated Filing Election or Notice of Revocation for a Selected Listed Financial Institution

Fill out this form if an investment plan manager and a group of two or more investment plans that are selected listed financial institutions want to make or revoke a consolidated filing election that allows the investment plan manager to file GST/HST returns for the group on a consolidated basis.
