EMPLOYER'S ANNUAL HARASSMENT AND VIOLENCE OCCURRENCE REPORT (EAHVOR)

Federally regulated employers must complete this report on or before March 1 of each year. This annual report records the total number of occurrences of workplace harassment and violence that were reported to the employer in the previous calendar year. The following information must be provided for each reported occurrence: Type of occurrence Location of occurrence The working relationship between the parties, and The method by which the resolution process was completed The EAHVOR can be submitted either by mail, email, Data Gateway, or by the ‘Submit’ button located on the PDF form.
