Report to the Head of Compliance and Enforcement Pursuant to Subsection 176(5) of the Canada Labour Code, Part III

This form will be used by federally regulated employers to report the number of hours worked in excess of the maximum hours per week (or more as per section 172 of the Canada Labour Code) by employees during the period covered by the permit.
