Notification of Fiscal Months

Fill out this form if you are a licensee under the Excise Tax Act (other than Part IX) or the Excise Act, 2001 and you want to notify the Canada Revenue Agency of your fiscal months or to request approval to use fiscal months that are not within standard guidelines.
