Fill out this form if you are an investment plan that is a selected listed financial institution and you are making or revoking an election under subsection 225.4(6) or (7) of the Excise Tax Act to have subsections 225.4(3), (4) or (5) not apply to a series of a stratified investment plan, a non-stratified investment plan, a pension entity of pension plan or a private investment plan.
