Application for a Social Insurance Number

If you are applying online or in-person at a Service Canada Centre, you do not need to complete this application form. To apply for a first-time Social Insurance Number (SIN), to obtain a confirmation of your SIN, or to modify your SIN record, complete this application form: Your application must be accompanied by original and valid identity documents.
