Non-Attending Out of Country/Out of Province Proctor Examination Request Form

Independent study option (ISO) students who are temporarily out of the province use this form to request an offsite exam writing. An approved test supervisor (proctor) must sign the form. The proctor is contacted and approved by the Distance Learning Unit.
