This form provides basic business information (business name, address and officers of the organization) along with the industrial sectors it operates in, total number of employees (full-time, part-time and temporary), geographical area and includes a statement for signature by a senior officer of the business, certifying the accuracy of the information in all Forms 1 to 6.
