Complaint Form – Monetary and Non-Monetary

This form is used by the public, excluding individuals employed as drivers working in the Road Transport – Trucking industry, to file a monetary or non-monetary complaint under the Canada Labour Code, Part III (Labour Standards) and must be submitted to start an investigation by a Labour Program inspector. This form replaces ESCD LAB1166 (2013-08-004) E.
