For persons residing in New Zealand to apply for a Canada Pension Plan Disability Benefit under the Agreement on Social Security between Canada and New Zealand. The Disability kit includes: New Zealand/Canada Agreement - Application for CPP Disability Benefits (ISP5053-NZL), Reference Guide and Medical Report for a Canada Pension Plan Disability Benefit (ISP2519-IO-NZL). Each form must be completed and forwarded to your nearest social security office (see Returning the Form).
