For persons residing in Saint Lucia to apply for Canada Pension Plan Disability Benefits under the Agreement on Social Security between Canada and Saint Lucia. The Disability kit includes: Application form, General Information and Guide, Questionnaire, 2 Consent for Service Canada to Obtain Personal Information forms and Medical Report. Each form must be completed and forwarded to your nearest Service Canada office (see Returning the Form).
