Germany/Canada Agreement - Application for CPP Disability Benefits

For persons residing in Germany to apply for Canada Pension Plan Disability Benefits under the Agreement on Social Security between Canada and the Federal Republic of Germany. The Disability kit includes: Application form, General Information and Guide, Questionnaire, 2 Consent for Service Canada to Obtain Personal Information forms and Medical Report. Each form must be completed and forwarded to your nearest Service Canada office (see Returning the Form).
