Consent to deduction and payment - Old Age Security

ISP has agreements with seven (7) Provinces (Newfoundland, Prince Edward Island, Nova Scotia, New Brunswick, Ontario, Manitoba, and Alberta) which allow, with client's consent, reimbursement of retroactive payments when their application for OAS benefits has been approved. This form is applicable only to Provinces which have an agreement with OAS.
