Vocational Confirmation/Activity Report - Canada Pension Plan Disability Vocational Rehabilitation Program

This form is to be completed by the Vocational Rehabilitation Service Provider to provide the Service Canada Vocational Rehabilitation Case Manager with a summary of occupational labour market research, recommended occupational options, and an analysis of a client’s vocational potential, which will help determine whether to proceed with Vocational Rehabilitation.
