Job Search Agreement – Canada Pension Plan Disability Vocational Rehabilitation Program

This form is to be completed by the Client to provide the Service Canada Vocational Rehabilitation Case Manager with an agreement on job search activities as outlined in the Intervention Phase of the Statement of Work.
