Individual Written Rehabilitation Plan (IWRP) – Canada Pension Plan Disability Vocational Rehabilitation Program

This form is to be completed by the Vocational Rehabilitation Service Provider to give the Service Canada Vocational Rehabilitation Case Manager a summary of the client’s rehabilitation plan and recommendations.
