Concept Application Form: Reaching Home - Community Capacity and Innovation (CCI) Funding

The funding program under which your organization is applying has specific eligibility requirements. The Concept Application Form should clearly show how your proposed project meets these requirements. Employment and Social Development Canada (ESDC) must receive the Concept Application Form by the closing date.
