Youth Employment and Skills Strategy Program – Participant Report (PR)

Purpose: The purpose of this form is to meet participant data reporting requirements specified in Schedule C of the Youth Employment and Skills Strategy Program contribution agreement. A completed Participant Report is required for each youth selected for the project. It must not be used to collect data directly from the youth. An optional Participant Information Template is available to obtain data from the youth.
